You are eligible for coverage for yourself (Member only) and your family from a minimum of $25,000 to a maximum of $200,000 in units of $25,000. You can apply at any time by completing the group benefit application form. You do not need to provide proof of good health for this coverage to take effect.

If you select family coverage

• your spouse will be insured for 60% of your coverage amount if you do not have dependent children or 50% if you have dependent children at the time of the claim
• each dependent child will be insured for 15% of your coverage amount if you have a spouse, or 20% if you do not have a spouse at the time of the claim.
